This form is used exclusively for financial aid purposes to ensure your aid and cost of attendance are accurate. It is separate from other housing forms required by different departments.

Important Notes:

• If this form is listed as a financial aid requirement, you must complete it to avoid delays in processing and receiving your financial aid.
• Submit this form if your housing status is incorrect to prevent errors in your financial aid awards.
• Completing this form updates your housing information for financial aid purposes only. It does not update your address or housing details with other campus departments.

Action Required:

• Notify relevant campus offices of any housing changes.
• Update your address on the MAX and/or SAM portals as needed.
