SMC Event Setup & IT Request Form

This form is ONLY for events held within the SMC.  Click HERE to request Tech Ops for events NOT in the SMC.

Timeframe:  This form should be submitted at least 2-weeks in advance of the scheduled meeting/event.  Requests made within less than 5 business days before the scheduled event cannot be guaranteed and are subject to the availability of staff and facility resources at the time.  

Cancellation:  Setup is free for internal events that include MU students; however, if the sponsoring organization cancels the scheduled event they must notify the appropriate setup crew at least 24-hours in advance to cancel requested setups.  Failure to do so may result in a $25.00 setup fee.

This form is for faculty and staff to request customized space setup, additional event resources, and Audio-Visual requests for spaces managed by SMC Operations and/or Campus Recreation. Please ensure you have reserved the space through Ad Astra before completing.
Student Organizations should NOT use this form and instead submit all event information through the Event Registration Process in Get Involved.
