Advanced Refund Request
If your financial aid exceeds your charges and you expect to receive a refund, you may request an advance of these funds to cover essential expenses like books, rent, or other immediate needs before your full aid is disbursed. An advanced refund is not additional aid. Click below to learn more.

APPEAL FORM (FEDERAL SATISFACTORY ACADEMIC PROGRESS)
The Federal Appeal Form is for students who have lost their financial aid due to not making Satisfactory Academic Progress (SAP) and had extenuating circumstances that caused them to not meet the standards. AUTHORIZATION FORMS
The Authorization to Release Information Form is to be completed by the student, permitting the Office of Financial Aid to speak with persons other than the student about his/her account. Grades and academic progress will never be discussed with individuals other than the student directly.

CONSORTIUM AGREEMENTS
The Consortium Agreement forms below are for students who plan to obtain their degree from Millersville University but are taking a course (or courses) at another institution.
- You must also complete the Authorization to Transfer Credit Form for the Registrar's Office and submit a copy to the Office of Financial Aid once it has been completed and signed by all parties. Consortium Agreements will not be processed until all documents, including your transfer of credit form, have been received by our office.
- If you are taking courses at another institution in a dual-degree program or a 3+1 program please contact the Office of Financial Aid at 717-871-5100 so the appropriate form may be sent to you.

ENROLLMENT FORMS
This form is for any student whose enrollment plans are different from what is listed on their MAX account. It is the student's responsibility to update the Office of Financial Aid on any changes to their enrollment. Please make sure to complete the change of enrollment form for the appropriate academic year. Make sure to enter all of the fields on the form; do not leave anything blank. You can complete this form and submit it to the Office of Financial Aid.

This form is to notify the Office of Financial Aid of changes in your housing status and/or your local address. This form is for Financial Aid purposes ONLY. You must also update any address changes online through your myVILLE Student Portal.

loan forms
The Loan Request Change form is used if you want to make any changes to your loans. This includes the Federal Direct Subsidized, Unsubsidized, Parent PLUS Loan, and/or Alternative Loans. If you have questions on how to complete this form, please contact the Office of Financial Aid at 717-871-5100.
The Private Loan Self-Certification form may be required for any private/alternative loans that are not certified by the Office of Financial Aid.

This form gives you specific information regarding the Direct Loan program and student eligibility.
PRIVATE LOANS FOR SPECIAL CIRCUMSTANCES
This is a list of Private Loan lenders that allow students to borrow loan money under certain special circumstances such as being enrolled in less than 6 credits, not making Satisfactory Academic Progress (SAP), non-degree students, and students with a past-due balance.

Summer Aid Request
Financial assistance is available for students who are attending classes during the summer semester; however, it differs from fall and spring aid due to its condensed timeframe and unique eligibility criteria. It also requires separate applications and processing. Click below to access the form and read the information and quick facts before submitting your application.

VA ENROLLMENT CERTIFICATION FORMS
Students must fill out a VA Enrollment Certification Form each semester after they register for classes. A separate form must be submitted for each semester you wish to receive benefits. VA Enrollment Certification Forms can be completed online via our StudentForms portal using the link below.

SATISFACTORY ACADEMIC PROGRESS POLICY (SAP)
SAP is used to define the successful completion of coursework to maintain eligibility for student financial aid. Federal regulations require the University to establish, publish and apply standards of your progress toward completion of your certificate or degree program. RETURN OF TITLE IV FUNDS POLICY
A Title IV Calculation is a federally mandated process by which a school is required to calculate the amount of federal funds to be returned for a Title IV federal aid recipient who withdraws or who ceases attendance during a payment period of enrollment.
